HOW WE WORK

INITIAL REFERRAL

Whether you have been referred to us by the Social Services Department or have come to us direct, information about you will be dealt with sensitively and in confidence. Prior to providing any services we will need to understand more about you and be sure that our services are suitable for your needs.

ASSESSING YOUR NEEDS

If you have been referred to us via the Social Services Department, they will provide us with a summary of your requirements called a Needs Assessment. If you have approached us directly, one of our specially trained staff will carry out this assessment which is client-focused and individual to your need.

SERVICE USER CARE PLAN

Having assessed your needs, we will prepare a Service User Care Plan for the care we expect to deliver. This will outline the services we will provide with details such as times of visits, special tasks to be performed, and the general objectives of the care provision.

REASSESSING & REVIEW

Over time, your needs may change. You may need more or less care, the type or pattern of service may have to be varied and new risks may become apparent. We will keep your needs under review and take decisions about your care together.
